To request credentials for any Northwestern sport, outside of football, for the 2022-23 season, please complete the below form to the best of your ability. If you are requesting on behalf of someone else, please fill out your information in the Requestor Section and indicate to whom the credential is for in the Credential Request Section (the name and affiliation you type in this location will appear on the credential).
