This "Credential" is issued by Nashville Predators for the sole purpose of providing facility access to an individual with a legitimate working function in connection with the event(s) for which Credential is issued (each, a "Game") at the venue(s) where such Games will be played (each, a "Venue") subject to the following conditions:

WARNING! DESPITE ENHANCED SPECTATOR SHIELDING MEASURES, PUCKS STILL MAY FLY INTO SPECTATOR AREA. SERIOUS INJURY CAN OCCUR. STAY ALERT AT ALL TIMES INCLUDING DURING WARMUP AND AFTER PLAY STOPS. IF STRUCK, IMMEDIATELY ASK USHER FOR DIRECTIONS TO MEDICAL STATION.

The bearer of Credential ("Bearer") assumes all risks incidental to (i) the game of hockey and/or in connection with the Game, including specifically (but not exclusively) the risk of injury by objects including but not limited to hockey sticks, pucks and other equipment, and (ii) the performance of Bearer's working function near, outside, and/or inside the Venue, including specifically but not exclusively, physical injury and illness (including effects related to human coronaviruses).

To the fullest extent permissible by law, Bearer agrees that National Hockey League and its constituent clubs and players, including Game participants, NHL Enterprises, L.P., NHL Enterprises Canada, L.P., the Venue, and their respective agents, representatives, affiliates, officers, directors and employees (collectively "NHL"), are not liable for injuries or illness suffered by Bearer whether resulting from Bearer's activities at the Game or Venue or otherwise. In consideration of benefits received, Bearer hereby releases NHL from any liability arising from, and agrees not to assert any claim on account of any damage, loss, injury, or otherwise in connection with the Game or Venue. Bearer shall indemnify, defend (if requested) and hold harmless NHL from any liability, loss, damage or expense caused by Bearer.

Bearer also grants NHL permission to record his/her image, name, voice, likeness, actions, and/or statements in any image, footage, or display, whether live or recorded, which may be taken at the Game or at the Venue, and to the use of same by NHL at any time, for any purpose (including advertising or promotional purposes), and in any media now known or subsequently developed without additional compensation.

Any unauthorized transmission, picture, or other depiction or description of game action, game information, player interview or other arena activity is prohibited without prior written approval of NHL. Bearer is also strictly prohibited from posting, sharing, transmitting, publishing, or otherwise distributing, including on social media or other digital platforms, images and/or copies of Credential in whole or in part.

Credential is for use solely in connection with Bearer's working function on behalf of Nashville Predators. While within the Venue, Bearer shall be subject to the policies and procedures of the NHL, the NHL member club, and the Venue as applicable. Credential is not transferable, may be revoked at any time at the sole discretion of Nashville Predators, and will automatically terminate if any term is breached.
