This "Credential" is issued by [NHL Club] to "Accredited Organization" for the sole purpose of providing facility access to an individual with a legitimate working function on behalf of Accredited Organization in connection with the event(s) for which Credential is issued (each, a "Game") at the venue(s) where such Games will be played (each, a "Venue") subject to the conditions below. Credential is not transferable and is solely for use by the individual identified on this Credential ("Holder"). Credential may be revoked at any time at the sole discretion of [NHL Club]. Failure to comply with any term of the Credential will result in immediate revocation.

Accredited Organization together with the Holder and each of their agents and personnel (collectively "Bearer"), by acceptance and use of Credential, each represent and agree that Bearer has a legitimate working function at the Game. Credential is for use solely in connection with Bearer's news and editorial Game coverage. The following is prohibited without prior written approval of National Hockey League (collectively "Unauthorized Use"): (a) Non-editorial, commercial or other unauthorized use of any transmission, picture or other depiction or description of Game action, Game information, performances, player interviews or other arena or Event activity, and (b) non-editorial or commercial use of any NHL (as defined below) mark or NHL club mark. To the extent Bearer participates in online activities, Bearer shall comply with NHL's Online Transmission Policy set forth on NHL.com. Bearer is strictly prohibited from reproducing, posting, sharing, transmitting, publishing, or otherwise distributing, including on social media or other digital platforms, images and/or copies of Credential in whole or in part.

Bearer assumes all risks incidental to (i) the game of hockey and/or in connection with the Game, including specifically but not exclusively the risk of injury by objects including but not limited to hockey sticks, pucks and other equipment, and (ii) the performance of Bearer's working function near, outside, and/or inside the Venue including specifically but not exclusively, physical injury and illness (including effects related to human coronaviruses).

Bearer agrees that National Hockey League and its constituent clubs and players, including Game participants, NHL Enterprises, L.P., NHL Enterprises Canada, L.P., the Venue, and their respective agents, representatives, affiliates, officers, directors and employees (collectively "NHL") are not liable for injuries or illness suffered by Bearer whether resulting from Bearer's activities at the Game or Venue or otherwise. In consideration of benefits received, Bearer hereby releases NHL from any liability arising from, and agrees not to assert any claim on account of any damage, loss, injury, or otherwise in connection with the Game or Venue. While at the Venue, Bearer shall be subject to the policies and procedures of the NHL, the NHL member club, and the Venue as applicable.

Bearer shall indemnify, defend (if requested) and hold harmless NHL from any liability, loss, damage or expense caused by or resulting from Unauthorized Use, the Bearer, the presence of equipment brought or used by Bearer, or the use of depictions or descriptions of matter other than the Game.

Bearer also grants NHL permission to record his/her image, name, voice, likeness, actions, and/or statements in any image, footage, or display, whether live or recorded, which may be taken at the Game or at the Venue, and to the use of same by NHL at any time, for any purpose (including advertising or promotional purposes), and in any media now known or subsequently developed without additional compensation.
