I hereby acknowledge that I have received a credential from The Los Angeles Lakers, Inc. ("Lakers") that permits me to enter the Crypto.com Arena (the "Arena") for 2023-24 NBA home games of the Los Angeles Lakers at which I am working (the "Credential"). The Credential is, and shall always be and remain, the property of Lakers.

I understand that the access granted by the Credential is to be used for WORK RELATED PURPOSES ONLY, and that the Credential is not to be used at Los Angeles Lakers games I may attend as a spectator or as a ticket to or for the purpose of entering the Arena for or during any other event held at the Arena. I understand and agree that the only way I am entitled to enter the Arena and view a game or event as a spectator is with a valid admission ticket, and I understand that if I do so I can only sit in the seat designated by that ticket. The Credential does not give me access to sit in an open seat or to enter any portions of the Arena during such games or events that are not open to persons possessing similar tickets to the game or event.

I understand that if I utilize or attempt to utilize the Credential to enter the Arena for any purposes unrelated to work or if I otherwise misuse the Credential in any manner, I will be subject to disciplinary action up to and including the possible termination of my employment and/or work assignment. Further, I also understand that allowing anyone else to use the Credential assigned to me is strictly prohibited and that failure to comply with this prohibition will also subject me to disciplinary action up to and including the possible termination of my employment and/or work assignment.

I further acknowledge that the Credential is to be surrendered immediately upon request by Lakers, and that it must be surrendered in the event of the termination of my employment and/or work assignment with Lakers.

I further acknowledge that if my assigned Credential is lost or destroyed, I will be charged $250 if a replacement is provided to me by Lakers and that if my Credential is stolen, my request for a new credential must include a police report demonstrating that I reported such theft to the police.

I further acknowledge and agree that my Credential must be surrendered immediately upon request by Lakers, and that it must be surrendered in the event of the termination of my employment and/or work assignment with Lakers.
